SELECTION PROCESS

Grant applications will be reviewed by the Horizon Academic Advisory Board, an independent advisory board made up of experts in the fields of glaucoma, cornea, and retina, with the assistance of the Allergan Grant Review Committee.* Applicants will be required to provide information about their institution and fellowship program, provide historical information about past clinical fellows from your institution, list research programs/projects currently ongoing in which an existing fellow is participating, and describe specific research programs/projects the Allergan Horizon Grant fellow will be conducting in the coming year. Grants will only be awarded to fellowship programs that conduct clinical research in the diagnosis or pharmacological treatment of glaucoma, corneal, and retinal diseases.

The Horizon Academic Advisory Board is composed of 3 advisors each in glaucoma, cornea, and retina. The advisors will each serve 2-year terms. Horizon Academic Advisory Board members do not participate in the evaluation of applications from their own institutions. Horizon Academic Advisory Board members serve in their individual capacities and any opinions/views expressed in connection with their service may not reflect the opinions/views of their employers or other affiliated institutions. Views expressed by Board members are not sanctioned or endorsed by their employers or affiliated institutions.

*If appropriate applications are not received in a specific disease category, the grant money will not be awarded.

PROGRAM ADMINISTRATION/STIPULATIONS

Allergan Horizon Grants are educational grants paid to the institution. The grants will be distributed to the institution during the summer. The institution is responsible for administering the funds in accordance with its prevailing procedures and policies, and the stated stipulations of this Allergan grant.

Use of funds: The Allergan Horizon Grant is intended primarily to support salary and research of the institutions’ fellows and shall not be used for overhead, indirect, or fringe costs. Research must be conducted by a US-based clinical fellow in an academic setting. Use of award funds toward direct expenses necessary for the proper operation of the fellowship program is also acceptable. This funding is being provided solely for educational purposes. At the close of the academic year, the institution must reconcile grant funds. Any unused funds must be returned to Allergan. This grant is not intended, nor should it be construed as, a reward for past or incentive for future decisions regarding the prescription, use, purchase, or recommendation of Allergan products. Allergan expects recipients and their institutions to exercise their best and independent professional judgment regarding the care and treatment of individual patients.

Outcomes: A research paper/manuscript must be submitted to Allergan at the conclusion of the academic year for the purpose of sharing the research outcomes as a result of receiving the award. Allergan reserves the right to request a live presentation to be given by the fellow or department chair/fellowship director at the conclusion of the academic year for the same purpose.

Rights to data: Applicant will have sole and complete rights (eg, publication, presentation, distribution, etc) to all data resulting from the research outcomes achieved as a result of receiving the award. Allergan, however, will be given the opportunity to review and provide input to applicant regarding consolidated data as contained in a manuscript, abstract, presentation, etc. Allergan reserves the right to address any factual inaccuracies with the publisher.

Transferability, forfeiture, and termination: Should the institution anticipate changes in their fellow’s program circumstances or situation for reasons including, but not limited to, discontinuation of the program, Allergan must be notified promptly. If forfeiture, temporary suspension, or termination of the grant is desired or warranted by Allergan or the institution, a written agreement of the same shall be executed, terminating the agreement. Institution will promptly return any unused funds to Allergan at that time.
